Thyme-Honey Cough Syrup
Thyme-honey cough syrup is one of the most evidence-supported herbal cough preparations available. A 2006 RCT found a thyme-based syrup produced comparable results to ambroxol, a commonly prescribed expectorant, in 361 patients with acute bronchitis. The thymol in thyme has antispasmodic effects on airway smooth muscle, and honey adds a demulcent coating layer with its own cough evidence. This takes about 20 minutes to make and keeps in the fridge for a week.
Instructions
- Rinse the fresh thyme under cold water. Add thyme and water to a small saucepan.
- Bring to a gentle simmer over medium heat, then reduce to low. Cover and simmer for 10 minutes.
- Remove from heat and steep, covered, for an additional 5 minutes to extract remaining volatile oils.
- Strain through a fine mesh strainer into a clean heatproof jar, pressing the herbs to extract the last of the liquid. Discard the spent herbs.
- Let the liquid cool until warm but not hot — below 104°F (40°C). Adding honey to hot liquid degrades its beneficial compounds.
- Stir in honey until fully dissolved. Add lemon juice if using. Stir again.
- Let cool completely, seal with a lid, label with the date, and refrigerate. Use within 1 week.
- Dose: 1 teaspoon as needed for cough, up to 4 to 5 times per day. Shake or stir before each use.
Why it works
Thyme contains thymol and carvacrol, volatile compounds that relax bronchial smooth muscle, reducing the spasm reflex that drives coughing. Thymol also has broad antimicrobial activity, which is why it appears as an active ingredient in commercial antiseptic products. Covering the pan while simmering and steeping traps volatile oils that would otherwise evaporate into the air rather than into your syrup.
Honey coats irritated mucous membranes and has independent cough evidence: a Cochrane review found honey reduced cough frequency and improved sleep in children with upper respiratory infections better than some OTC preparations. The combination gives you both a smooth-muscle relaxant and a throat-coating demulcent in a single dose.
What the evidence says
The key study is Kemmerich et al. (2006, Arzneimittelforschung): a controlled trial of 361 adults with acute bronchitis comparing a thyme-ivy herbal syrup to ambroxol hydrochloride, a standard mucolytic expectorant. Both groups showed significant cough reduction over 11 days, with the thyme group showing non-inferior results and similar tolerability. A 2007 follow-up by Kemmerich confirmed these findings in a larger population.
The studied product contained both thyme and ivy leaf (Hedera helix). This recipe is thyme-only. Thyme is the primary active component — thymol's antispasmodic and antimicrobial properties are well-documented — but the combination product has not been directly tested against thyme alone in the same trial design.

- Keep the simmer gentle — a hard boil drives off the aromatic thymol compounds you want in the syrup.
- Always wait for the liquid to cool before adding honey. Heat above 104°F (40°C) degrades honey's beneficial enzymes and antimicrobial compounds.
- Fresh thyme gives a brighter, more aromatic syrup. Dried thyme is more concentrated — use about half the volume.
- A small squeeze of lemon brightens the flavor and adds a little vitamin C.
- For a simple on-demand version, steep fresh thyme in hot water like tea, strain, add honey, and drink immediately — no stovetop required, though less concentrated.

- Does this work as well as pharmacy cough syrup?
- The Kemmerich RCT compared a thyme-ivy preparation to ambroxol, a mucolytic expectorant, not to dextromethorphan or codeine-based suppressants. Results were comparable to ambroxol. This syrup has not been directly compared to OTC cough suppressants in clinical trials.
- The study used thyme and ivy together — does this recipe work without ivy?
- The Kemmerich study used a combination product. Thyme was the primary active ingredient, and thymol specifically has documented antispasmodic and antimicrobial effects. Ivy leaf (Hedera helix) is a separate plant that is not in this catalog. The syrup here is thyme-only, which is consistent with the traditional European thyme syrup used before combination products existed.
- How long does the syrup keep?
- Refrigerated up to 1 week. Honey is a preservative but this is a dilute preparation in water — it is not shelf-stable. Discard if it smells fermented, cloudy, or off. Label the jar with the date so you know when to replace it.
- Can children use this?
- The honey makes this unsuitable for infants under 12 months due to botulism risk. For older children, the thyme tea component is generally considered safe, but check with a pediatrician before giving herbal preparations to young children or children with asthma or reactive airway disease.
- When should a cough get medical attention?
- Seek care for cough lasting more than 3 weeks, high fever, coughing up blood, shortness of breath, wheezing, chest pain, rapid breathing, significant weight loss, or known asthma, COPD, or heart disease that makes any cough change alarming.
Safety: Never give honey to infants under 12 months due to botulism risk. This is a comfort preparation for mild-to-moderate acute cough from upper respiratory infection. It does not treat asthma, pneumonia, COPD, whooping cough, or croup.
